Fundamentals is a 5 star full service early childhood learning and preschool center. We provide care and early learning for the youngest of learners. Fundamentals consists of 2 buildings, an Infant/Toddler House and our Preschool Center. Our Infant /Toddler house serves ages 6 weeks-30 months and our Preschool Center serves ages 30 months-5 years. We have two playground areas for each age group to explore, play, and learn in. We also have an outdoor classroom with a garden, construction zone, story area, music wall, art area, and mud kitchen.
Fundamentals offers individualized programs for each of our little learners. Our approach is based on developmental skills and readiness instead of chronological age. As children are ready to transfer from one program to the next our staff will work closely with our families to create a successful transition.

- The Harvest Food Hub, an EDA University Center project, is an aggregator and online marketplace of locally grown and produced food. We make it possible for farmers to work together to meet the growing demand for local, sustainable food in San Juan County. Farmers benefit from the marketing, selling, and delivering of their produce and prepared foods. The Harvest Food Hub brings fresh, healthy, locally grown and produced food to you, the buyer, in a cost-effective manner on a weekly basis.

San Juan Regional Medical Center is committed to providing the support services and education expecting parents need to make sure their baby gets the best start in life. We offer a series of complimentary Childbirth Preparation classes that cover a range of topics important for expecting mothers and their families.
All classes are free of charge and are offered monthly, some classes can also be taken by appointment.

Eligibility:
This program serves individuals whose disability prevents them from using the local public transportation.Reservations for Paratransit and Dial-A Ride services can be made any time during system operational hours and on Saturday for the following Monday. Reservations may be made up to 30 days in advance. Paratransit reservations may be made for the following day service. -
In prenatal yoga sessions, we will focus on learning beneficial movements, positions, helpful breathing techniques, massage, and relaxation in order to achieve a more comfortable pregnancy and labor. We will also touch on prevention of potential postpartum complications, such as diastasis recti, prolapse, and depression and/or anxiety. You will learn how to bond with your unborn baby and other pregnant moms, creating a more soulful and meaningful birth.
During the postpartum phase of pregnancy, you will learn valuable ways to heal, protect, and strengthen your core and pelvic floor and safely return to your previous exercise and activity levels. I will work with you and your body at your own comfort levels to facilitate movement and flexibility, as well as practice techniques to manage and reduce stress and anxiety. This practice helps to heal from the physical and emotional traumas from birth, and gives you the option to share your birth experience within a safe and supportive community.

- The purpose of NMBTF is to bridge the gap in breastfeeding/chestfeeding disparities by ensuring all families have the support they need to reach their breastfeeding/chestfeeding goals. A breastfeeding/chestfeeding culture includes all forms of feeding human/mother’s milk, including pumping, donor milk feeding, and milk sharing.

- Kiwanis Clothes for Kids Memorial Day Riverfest Duck Race is an annual event that partners with the River Reach Foundation. Kiwanis share of the proceeds benefit Kiwanis Clothes for Kids.

Kids Kollege is dedicated to providing young people with exciting learning opportunities that allow students to explore, expand and experience areas of interest. Kids Kollege is a summer enrichment program for children and teens between the ages of four and fifteen.
San Juan College offers many year-round activities for kids such as birthday parties at the climbing wall, field trips to the Sherman Dugan Museum of Geology and a variety of non-credit classes.

Cenikor is an evidence-based treatment provider accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). With our personalized programs, clients can adjust their recovery methods as needs change. Some individuals are best served with short-term residential programs, while others need the convenience and flexibility of our outpatient facilities. Cenikor works with each person to find the right recovery treatment plan.
It is not unusual for an individual to start out in a detox or withdrawal management program, then transition to short-term residential or outpatient care. No matter where a person starts, Cenikor is dedicated to finding the right level of care. Once sobriety is achieved, Cenikor continues its support through sober living opportunities, an alumni support network and more across its network of locations.

The Supportive Services for Veteran Families program helps stabilize housing for very low-income veterans and their families who are homeless or at risk of homelessness.
Our Homeless Veterans' Reintegration Program focuses on placing veterans that are currently experiencing homelessness or at risk of homelessness into long-term employment, as well as provides social services.
Get job search assistance in our GoodJobs program. It's designed to connect individuals with employers throughout the community and help them find long-term employment that meets their needs, abilities, and interests. Are you a first-time job seeker? Childhaven offers Private Placements to support parents and guardians in need of temporary, safe placement for their children.
Parents and guardians who are experiencing temporary barriers in providing care for their children may voluntarily place their children at the Childhaven Children’s Shelter. Children’s health and safety situations that may warrant voluntary placement include parent/guardian’s:

The main focus of my practice is public speaking. As an Advanced Toastmaster and member of the National Speaker’s Association, I am in my element when addressing an audience. The FIT Program partners with your family to understand your child’s strengths and needs. We recognize that you are the most important person in your child’s life. Your knowledge of your child will help guide the Early Intervention process.If your child is eligible for services, you , your FSC and your Early Intervention provider(s) will work together to develop a plan of supports and services that are a good fit for your family. Your Individualized Family Service Plan (IFSP) will include goals, strategies and ideas that you can use every day to support your child’s learning and development.

The Volunteer Incentive Program partners with local youth organizations, including athletic, academic, religious, and social groups, by donating $10 per student per hour spent volunteering (up to $600 per organization per year). The youth organizations choose their own volunteer activity and request approval from VIP. Upon completion, they are compensated out of our General Fund, which is funded by our donor pool. Donors also have the option to designate their contribution for specific youth organizations, in which case the maximum amount available can be raised (great for larger organizations!)

It’s My Very Own (IMVO), a faith-based, voluntary organization, seeks to make kids’ lives a little brighter by working with communities to provide displaced children with Bags of Love that meet their immediate needs.
Each bag of love includes a handmade quilt, stuffed animal, toys, books, and personal items. The bags are age appropriate and are delivered to the local agencies charged with removing children from unsafe homes.

The McKinney-Vento Act is federal legislation that states and school districts follow to try to keep students experiencing homelessness connected with school. Every school district has a named homeless liaison and they can be in communication with each other. Their goal is to remove barriers to enrollment and success in school for students in unfortunate living situations.

We are a homeschool support group that offers weekly park days and various field trips and other events throughout the year.

The San Juan County Partnership Housing Assistance Program helps those who are experiencing homelessness and those in danger of becoming homeless with supportive services such as:
- Outreach to identify, coordinate and refer people and families experiencing homelessness to appropriate service providers while addressing their housing issues
- Financial assistance, which may include utility deposits, security deposits, rental assistance, and eviction prevention
Some of the various housing assistance programs include:
- Homeless prevention funds that provide assistance with paying arrears and possibly 3 months rental assistance, based on funding availability. Families need to have an eviction notice or a 3-day notice of nonpayment. The rental has to be under San Juan County’s FMR (Fair Market Rent) and has to pass inspection. There are income limits.
- Rapid Rehousing funds provide assistance with security/utility deposits and possibly 3 months rental assistance based on funding availability. Families have to be considered literally homeless, for example, coming from shelters, DV shelters or living in uninhabitable living places. There are income limits.
- CDBG funds through the City of Farmington provide security/utility deposits and first month rent, or pro-rated month, depending on when families move into the rental. Families have to be considered literally homeless. With this funding, HUD income limits can be exceeded up to a certain amount. This funding only serves clients within the city limits of Farmington.
- Permanent Supportive Housing (PSH) can provide security/utility deposits and 6 months rental assistance. The Head of household has to be considered disabled and chronically homeless. Chronically homeless means that the head of household has been continuously homeless for 1 year or has had 4 episodes of homelessness within 3 years that total 12 months. This funding has income limits.

Four Corners Home for Children
Four Corners Home for Children serves people in the Four Corners region of New Mexico, Arizona, Utah, and Colorado. Since 1953, we have helped meet the physical, emotional, educational, and spiritual needs of Navajo children and their families. Our mission as an organization is to help those we serve through programs that instill hope, restoration and Christian values.
Four Corners Home for Children operates a campus school, called Four Corners Academy for Excellence. The school serves students in kindergarten through eighth grade and is taught by our Director of Education and her Educational Assistant.

Farmington Preschool Academy West is a NM Public Education funded (NM PreK) site that serves Farmington students who are typically developing as well as including students with IEPs during their year immediately preceding kindergarten. Students must be four years old on or before August 31st of the current year and reside in Farmington Municipal School District. Waivers are considered on a case-by-case basis. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

Farmington Preschool Academy East enrolls students ages three to five who have a documented educational need for special education services. These students have Individualized Education Programs (IEPs) with goals that are written in the area(s) in which s/he qualifies. Peer students who model behavior and language also attend. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

Those who benefit from the Partnership are community agencies and organizations, government and private business, schools, families, and the community at large.
Prevention activities work to educate and support individuals and communities to prevent the use and misuse of drugs and the development of substance use disorders. Substance use and mental disorders can make daily activities difficult and impair a person’s ability to work, interact with family, and fulfill other major life functions. Mental and substance use disorders are among the top conditions that cause disability in the United States. What do we do?
The HCAP helps people who do not have medical insurance with their medical bills. There is an application process. The SJC HCAP provides assistance for medical, dental, prescriptions, mental health, and behavioral health.
Individuals may apply directly to San Juan Regional Medical Center or PMS/Farmington Community Health Center.
To qualify, you must meet these criteria:
- You must apply within 180 days of incurring your medical bills.
- You must be a resident of San Juan County for at least 90 days.
- Non-citizens can apply but must have "Legal Permanent Resident" status.
- There is an income limit - see HCAP Program Information for details.
- There is a limit on your personal assets - see HCAP Program Information for details.
- You must have applied for Medicaid and not qualified (we can help you with these forms)

Roundtree Children’s Developmental Services is a contract provider with the New Mexico Family Infant Toddler (FIT) Program. The FIT Program provides supports and services to children who have or who are at risk for a developmental delay, and their families. The Roundtree team has 100+ years of combined experience working with children and families to enhance your child’s learning and development.
Throughout the years, the delivery of early intervention has evolved to family guided services supporting your child’s learning and development in their natural environment. Staff partner with families to develop goals and strategies that are easily integrated into the family’s daily routines.

Project Shield is committed to helping developmentally disabled adults in San Juan County live more meaningful lives. The goal is to provide individuals with the greatest opportunity to learn, grow, and explore their communities.
Customized Community Supports
The service focuses on maximizing independence, creating interpersonal relationships, establishing natural supports, and participating in the local community. These supports are based on individual choice of outcomes and then designed to measure progress specified in their Individual Support Plan.
Project Shield assists individuals with disabilities to increase economic independence, self-reliance, and growth within a career. Based on the individuals' career plan, they will work with a job coach during the job search process, the application and interview process, and job improvement and growth. We provide job coaches, transportation, assistive technology, and specialized job training.

Early childhood program accredited by NAEYC Accreditation of Early Learning Programs have voluntarily undergone a comprehensive process of internal self-study and continuous quality improvement.
- Early Head Start: This federally funded program promotes academic readiness by helping children through their social and cognitive development by employing educational, health, nutritional, social, and other services. The program also helps parents reach their full potential for their personal, educational, or vocational goals. Early Head Start is available to pregnant women and their children between the ages of 6 weeks and 3 years old.
- Head Start: To enroll in Presbyterian Medical Services’ Head Start center-based programming, children must turn 3 prior before August 31/September 1 of that academic year, and the maximum age is 5 years.
- Pre-K: Our Pre-K program consists of direct services to 4-year-old children who will attend kindergarten the following year, providing developmentally appropriate activities for parental involvement. New Mexico Pre-K is a voluntary program funded by the state of New Mexico. It is designed to increase access to high-quality pre-K programs, help children through developmentally appropriate services, and it focuses on academic readiness.

- Parent Education/Home Visiting: This Early Childhood Service is designed for parents and guardians of children from birth through 5 years of age. This includes adoptive parents and teen parents, as well as expectant parents. The goals are to provide educational services and child development guidance, to ensure babies are born healthy, and to keep children safe and sound. Children’s psychological, emotional, and mental health are all addressed and parents are taught how to nurture their children through community support to ensure children are ready for school. This program is built on the belief that a parent is their child’s first and most important teacher.

We provide Sexual Assault Nurse Exams (SANE – medical/forensic exams), rape crisis advocacy, and counseling. Our staff also works in the community to prevent sexual assault through multi-session, education-based programs with specific groups. This education includes awareness programs, including ‘Safe Dates’, a healthy relationship curriculum, geared for high school students.
SAS will be will you throughout every step of your recovery process, from an exam to counseling, and facilitation of legal services to pursue the person who assaulted you. Established in 1978, Family Crisis Center (FCC) is a 501(c)(3) agency and comprehensive domestic violence service provider, offering a broad range of services geared toward response to the crisis of domestic violence. Services are provided at two facilities including Marge's Place, a protective shelter for victims of domestic violence and their children, and our Counseling Center which provides services and support to victims, child witnesses, and offenders.
Incorporating a trauma lens approach allows FCC to address the physical and emotional needs of domestic violence survivors, support the healing and recovery process, and empower survivors to take positive steps toward their own well-being and the well-being of their children.

DNA People’s Legal Services is a 501(c)(3) nonprofit legal aid organization that provides free civil legal services to low-income people who otherwise could not afford to hire an attorney. DNA provides legal assistance, advice and representation in U.S. and tribal courts, promotes tribal sovereignty, and offer community education programs that promote greater understanding of the law. DNA’s services have helped people living in poverty use existing policies and laws to protect their property and assets, stay safe from physical, mental and financial abuse, avoid exploitation and safeguard their civil rights.
