We have renamed the new center “Four Corners Cancer Center” to reflect the reality that San Juan Oncology Associates is the epicenter for treatment of cancer and blood-related diseases for more than four states. In tandem with San Juan Regional Medical Center’s Radiation Oncology Service, we provide a complete range of diagnostic/genetic diagnostic, chemotherapy, immunotherapy, radiation, and support services. As the only certified and highly experienced center for clinical trails in our complex specialty, we also welcome patients from many parts of the United States who are referred here for the state-of-the-art trials that offer the latest and often superior treatments for patients who respond poorly to standard treatments for their particular form of disease.
San Juan Oncology Associates’ physicians, nurses and Clinical Research Coordinators are highly trained, experienced and certified oncology/hematology specialists. They regularly attend national conferences to remain current in new developments and to reinforce professional interactions with oncology and hematology professionals in specialized centers throughout the United States. Should a patient require a procedure or treatment not available at our center, our physicians employ these ties as well as their comprehensive knowledge of current oncology and hematology literature to refer the patient to an appropriate center. Additionally, our physicians are fully credentialed and offer both inpatient and out-patient treatment.

WHO/WHAT WE ARE
Vdex Diabetes is a group of medical providers focused on better control of blood sugar in people with diabetes and prediabetes. It is the failure to adequately control blood sugar that leads to the complications of the disease, and those complications are painful, expensive and ultimately, life-threatening.
A HIGHER STANDARD
As we, the Vdex Founders, surveyed the landscape of diabetes care in 2015, one point became clear: there were lots of treatments and surprisingly poor results. We set out to do better. We could see the obvious need for a higher standard of care if one were to truly “control” the disease and its effects.
A DIFFERENT MODEL
We also noticed flaws in the model through which care was delivered to patients. That model left patients uninformed, with inadequate control and unsatisfied. Doctors were overburdened dealing with a burgeoning patient population experiencing poor outcomes and increasing complications. Insurance companies were facing ever-increasing costs of care with decreased levels of patient satisfaction. It was a perfect storm of negativity. It had to change.
FRESH THINKING – PHYSICIANS AND NONPHYSICIANS
Vdex brought fresh thinking to the problem. The Founders included both physicians and as important, nonphysicians. Each started from scratch. Instead of asking, “How do we improve the current system?” we asked, “What is the best system to produce the best results for patients?” We then built that.
DIFFERENT THERAPEUTIC AGENTS
We looked at the many therapies available to control blood sugar and asked, “Which work best?” Surprisingly, many of the therapies being used were little more than bandaids on the problem. They really didn’t get the job done long term, and worse, may have created a false sense of security in patients.
EARLY RESULTS ENCOURAGING
From early 2016 we began rigorous testing of our thinking both in terms of the therapies used and the process of delivering care. The early results were better than we had dared to hope. We saw patients getting better control of their blood sugar, with less effort, with greater satisfaction, greater compliance, and greater safety. This spurred us to go further.
THE VDEX STANDARD – 6.2
The Vdex Standards for control of diabetes are higher than the industry standards. While the American Diabetes Association target for blood sugar control is an HbA1c of below 7, we at Vdex strive for a target of 6.2 and below. Why? At 6.2 and below, most patients will have minimal to no complications from the disease.
While the ADA encourages providers to “minimize” hypoglycemia, the most feared short-term complication of treatment, Vdex providers are guided to “eliminate” hypoglycemia.
THE DAWN OF A NEW ERA OF TREATMENT
At Vdex we believe we sit at the dawn of a new era in the control of diabetes. We believe, for many, if not most, patients, we can so control the disease that it can be reduced from being a killer to a mere nuisance. We believe diabetes care can go from being one of medicine’s great failures, to a resounding success.

Families FIRST is a case management program of the New Mexico Department of Health, Public Health Division, that provides perinatal case management to Medicaid eligible pregnant women and children 0-3 years old. See website for information about this program. Clients receive the following services:

-Presumptive Eligibility and/or Medicaid On-Site Application Assistance;
-An assessment of the client’s medical/psychosocial strengths and risk factors;

The New Mexico Workers' Compensation Administration's (WCA) mission is to assure the quick and efficient delivery of benefits to injured workers at a reasonable cost to employers. The WCA works to maintain a balance between the interests of workers and employers, and maintains jurisdiction over most New Mexico businesses.
Injured workers who file for workers' compensation are central to all workers' compensation transactions. When injured in the workplace, they have the right to reasonable medical care, and sometimes, wage replacement.

We lovingly operate support groups and educational group interventions to encourage conversation about difficult topics. We offer a food assistance program, on-site medical clinic, transportation assistance for medical appointments, health insurance navigation and more.
We run stigma-free syringe exchanges, free HIV, syphilis and hepatitis C testing without any judgement, and a PrEP linkage program that guides people in need to the prevention methods that work for them. Our MAT (medication assisted treatment) linkage helps healthcare providers be able to use evidence-based treatments for people who use substances while also providing people who use substances information they can use to find treatment.
We operate housing assistance for qualifying individuals throughout New Mexico to see people moving from the streets into homes that foster healthier living opportunities. Funding from federal and state sources allows applications to receive rental assistance, utility assistance, and more.

- A residential care for older adults who need help with normal daily activities.
- Seniors in assisted living receive personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting as needed so that they’re able to live as independently as possible.

Preventative Care: Children’s physicals, Adult physicals, immunizations. Women’s Healthcare: annual exams, diagnosis, and treatment for gynecologic issues. Management of Chronic Medical Conditions: High blood pressure, high cholesterol, diabetes, digestive disorders, allergic reactions, asthma, headaches, arthritis, skin disorders, thyroid abnormalities. Diagnosis and Treatment of Acute Mental Illness: colds and flu, respiratory infections, throat and ear infections, bone and joint pain and injuries, burns and lacerations, gastrointestinal infections, individual counseling, family counseling, marriage and couples counseling.

Crisis intervention, comprehensive assessments, individual and family counseling, psychiatric consultation and medication, substance abuse programs, educational groups for a variety of problems, and additional community support programs for those with serious, chronic mental health conditions. Additional Veterans Support: Services in support of veterans and their families, including substance abuse, depression, anger, domestic violence, and social and economic challenges.

Service Provided:
IMMUNIZATION CLINIC: held in Bloomfield, call for appointment.
FAMILY PLANNING SERVICES: held in Bloomfield and based on a sliding scale fee. Offering annual exams, pap smears, birth control methods, counseling, pre-pregnancy planning, pregnancy tests, and emergency contraception. Call for appointment.
SEXUALLY TRANSMITTED DISEASE SCREENING AND TREATMENT: call for appointment.
HIV TESTING: Confidential or anonymous screening, treatment, and counseling. Call for appointment. TUBERCULOSIS, SCREENING AND FOLLOW-UP: We screen contacts of active cases of tuberculosis, and foreign-born individuals. Call for appointment.
FLU SHOTS: Target population is individuals over age 65 and/or people with chronic illness. Held in late fall in Bloomfield, Aztec and Blanco.
COMMUNICABLE DISEASE FOLLOW-UP: We follow up on cases of
reportable diseases such as salmonella, hepatitis, whooping cough, measles, and tuberculosis. Services are available to individuals with reportable diseases.
WOMEN, INFANTS, AND CHILDREN (WIC) NUTRITIONAL PROGRAM: Services are available by appointment and eligibility is based on family income and size. We serve women with children up to age 5, pregnant women, and postpartum women. Families FIRST Program: Families FIRST is a free program offered by the New Mexico Department of Health. The program provides case management services by registered nurses for Medicaid-eligible pregnant women and children up to 3 years of age. Families FIRST improves pregnancy outcomes and promotes healthy children and families by connecting clients with medical, social, and educational resources in the community. Families FIRST helps with obtaining: Prenatal care, Medicaid, SNAP, TANF and WIC benefits, childbirth and parenting classes, car seats and transportation to medical appointments and referral to community resources. -
Available to: Birth to 21 and adults with cystic fibrosis or metabolic disorders
Services Provided: CMS offers care coordination, specialty clinics and medical coverage for persons ages birth to 21, with chronic illnesses or medical conditions that require surgical or medical treatment and limit their activity. Eligibly conditions include diabetes, cardiac, cancer, cleft lip and palate, among many others. Specialty clinics are available by referral for asthma, cleft lip and palate, genetics, metabolic disorders, neurology, endocrinology nephrology, and gastroenterology. There is a financial eligibilityrequirement for medical coverage, except for diagnostic services and Adult Cystic Fibrosis. Medical coverage is further limited to persons who are not Medicaid-eligible.
