The Kirtland Growers Market's mission is to provide a weekly connection point for local growers and makers to sell their fresh produce, processed foods, and land-based craft items to the local community.  Market is held on Mondays 5-7pm, July - September at the Kirtland Town Hall Parking lot (43 Rd 6500 Kirtland NM)

The main focus of my practice is public speaking. As an Advanced Toastmaster and member of the National Speaker’s Association, I am in my element when addressing an audience. It is my great joy to provide participants with useful, practical tools, tips, and techniques to help them enjoy healthy relationships both at work and at home. The FIT Program partners with your family to understand your child’s strengths and needs. We recognize that you are the most important person in your child’s life. Your knowledge of your child will help guide the Early Intervention process.

If your child is eligible for services, you , your FSC and your Early Intervention provider(s) will work together to develop a plan of supports and services that are a good fit for your family. Your Individualized Family Service Plan (IFSP) will include goals, strategies and ideas that you can use every day to support your child’s learning and development.

The Self Help Center was created to assist people in the community, who wish to represent themselves in court.

The role of staff in the Self-Help Center is to provide basic, procedural information. They can not advise you what to file or how to proceed in a case, and can not provide any form of legal advice. They can provide information on other resources in the community or throughout New Mexico. Please be courteous and patient to staff and respect the limits on what they are able to do for you.

• They CAN give you court-approved forms, some forms might not be available for all legal issues.
• They CAN answer questions about what is being requested on a form.
• They CAN tell you what certain legal words mean.
• They CAN give you information on a court case, unless it is not available by law.
• They CAN give you general information on court rules, procedures and practices.
• They CAN give you court calendars and information on how to schedule a hearing.

Why We Developed This Program

The Adult Inpatient and Outpatient program that provides individualized treatment for adults who suffer from emotional, behavioral or mental health disorders.

The inpatient unit offers acute inpatient treatment for individuals with a primary psychiatric diagnosis. The outpatient program provides an intensive outpatient treatment Monday through Friday in the least restrictive environment and patients are able to return to their homes each evening. Medication Management Services are also offered.

How Do I Get Into The Program

Any concerned individual may refer themselves or someone else to the program. An initial clinical assessment is provided at no cost to determine the need for physician evaluation.

If you or a family member feel overwhelmed, unable to cope, extremely anxious, or have thoughts of wanting to harm yourself or someone else, please come to the Emergency Department.

A Community Health Worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served.

Community Health Workers become a part of the patient's care team by addressing any challenges they face with regard to their own health or that of their family.

All community members are served. They do not have to be a patient of GCRMC.

Families FIRST is a case management program of the New Mexico Department of Health, Public Health Division, that provides perinatal case management to Medicaid eligible pregnant women and children 0-3 years old. See website for information about this program. Clients receive the following services:

-A case intake to ascertain the client’s eligibility and to gather information for the purposes of care coordination;
-Presumptive Eligibility and/or Medicaid On-Site Application Assistance;
-An assessment of the client’s medical/psychosocial strengths and risk factors;
-A plan of care that links clients with resources that address the problems/needs identified;
-Standardized health education and literature related to pregnancy, birth, newborn care and child development;
-Referrals to appropriate community resources;
-Monitoring and follow-up with clients to ensure that they receives the recommended services;
-A reassessment to evaluate the status of problems identified, and to identify additional areas of concern or risk.

Four C Inspire is an in-school mentoring program the helps students discover their value, find purpose, and build strength.  Using volunteers and a well-deigned program with a proven track record, this program has helped high-risk or traumatized students succeed in school.  Four C Inspire was a valuable partner to Aztec High School  after the tragic 2017 in-school shooting and the death of two students.

The Volunteer Incentive Program partners with local youth organizations, including athletic, academic, religious, and social groups, by donating $10 per student per hour spent volunteering (up to $600 per organization per year). The youth organizations choose their own volunteer activity and request approval from VIP. Upon completion, they are compensated out of our General Fund, which is funded by our donor pool. Donors also have the option to designate their contribution for specific youth organizations, in which case the maximum amount available can be raised (great for larger organizations!)

It’s My Very Own (IMVO), a faith-based, voluntary organization, seeks to make kids’ lives a little brighter by working with communities to provide displaced children with Bags of Love that meet their immediate needs.

Each bag of love includes a handmade quilt, stuffed animal, toys, books, and personal items.  The bags are age appropriate and are delivered to the local agencies charged with removing children from unsafe homes.

Our mission is to provide a strong Recovery Oriented System of care for the Homeless, Veterans, Domestic Violence Abuse Victims, At-Risk teens as well as those exiting incarceration and those with mental and behavioral health challenges. We envision a world in which every person lives in a safe, caring community. The under individualized resident services includes housing, financial management, employability skills with case management, nutrition, family reunification, health and hygiene, life skills, self-esteem & self-empowerment, individual/group/peer support counseling, utilizing our collaborative network we refer residents to additional local, state and federal resources.

Double up with friends or relatives because you cannot find or afford housing

In an abandoned building, trailer, or other inadequate accommodations

We work with schools and their students in a classroom, small group, or one-on-one setting.  Our mission is to help students process their emotions and feelings through art and other creative expressions. We also work with couples and small groups outside of the school setting.

Inspire Heart Inc, is dedicated to enhancing the mental and physical well-being of individuals, communities, and to facilitate the discovery of one’s value and purpose through engaging in art therapy and creative expression.

We are a homeschool support group that offers weekly park days and various field trips and other events throughout the year.

The San Juan County Partnership Housing Assistance Program helps those who are experiencing homelessness and those in danger of becoming homeless with supportive services such as:

• Outreach to identify, coordinate and refer people and families experiencing homelessness to appropriate service providers while addressing their housing issues
• Financial assistance, which may include utility deposits, security deposits, rental assistance, and eviction prevention

Some of the various housing assistance programs include:

• Homeless prevention funds that provide assistance with paying arrears and possibly 3 months rental assistance, based on funding availability. Families need to have an eviction notice or a 3-day notice of nonpayment. The rental has to be under San Juan County’s FMR (Fair Market Rent) and has to pass inspection. There are income limits.
• Rapid Rehousing funds provide assistance with security/utility deposits and possibly 3 months rental assistance based on funding availability. Families have to be considered literally homeless, for example, coming from shelters, DV shelters or living in uninhabitable living places. There are income limits.
• CDBG funds through the City of Farmington provide security/utility deposits and first month rent, or pro-rated month, depending on when families move into the rental. Families have to be considered literally homeless. With this funding, HUD income limits can be exceeded up to a certain amount. This funding only serves clients within the city limits of Farmington.
• Permanent Supportive Housing (PSH) can provide security/utility deposits and 6 months rental assistance. The Head of household has to be considered disabled and chronically homeless. Chronically homeless means that the head of household has been continuously homeless for 1 year or has had 4 episodes of homelessness within 3 years that total 12 months. This funding has income limits.

Four Corners Home for Children

Four Corners Home for Children serves people in the Four Corners region of New Mexico, Arizona, Utah, and Colorado. Since 1953, we have helped meet the physical, emotional, educational, and spiritual needs of Navajo children and their families. Our mission as an organization is to help those we serve through programs that instill hope, restoration and Christian values.

Four Corners Home for Children operates a campus school, called Four Corners Academy for Excellence. The school serves students in kindergarten through eighth grade and is taught by our Director of Education and her Educational Assistant.

The New Mexico Home Fund helps renters and homeowners in New Mexico pay for their rent, mortgage, and utility bills. The program is funded by the Federal Government and administered by the State of New Mexico. The program brings two together the Emergency Rental Assistance Program (formerly RentHelpNM.org) and the Homeowner Assistance Program.

This NM Home Fund provides assistance for people who need some extra help due to the COVID-19 pandemic. Provided assistance does not need to be repaid. It provides support to households so that they can stay in their homes during difficult times.

Over $200M is available to support NM Renters and $55M is available to support homeowners. Both programs will run until September 2025 or until program funds are exhausted.

Farmington Preschool Academy East enrolls students ages three to five who have a documented educational need for special education services.  These students have Individualized Education Programs (IEPs) with goals that are written in the area(s) in which s/he qualifies.  Peer students who model behavior and language also attend.  Waivers are considered on a case-by-case basis. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

The New Mexico 5-Actions Program™ is a new way to access care on your terms from the privacy of your home so you can start living your best life.

The 5-Actions Program™ includes over 100 videos, 3-5 minutes in length, organized into learning modules. In total, there is about 6.5 hours of engaging content. Most videos also include links to additional resources such as TED Talks, books, and downloadable tools.

The program has extensive self-screening and assessment tools to help you identify problems with substance use and/or behavioral addictions in your life. The tools are not diagnostic, but may help you identify issues that contribute to your ongoing struggles. And if indicated, they will encourage further evaluation by treatment professionals.

It also includes access to free monthly webinars. Learn about emerging research in addiction, getting started using NM 5-Actions, indigenous healing practices, help for a loved one struggling with addiction, and more. The on-demand webinar library lets you listen and learn anytime at your convenience.

The Office of Diné Youth is committed to enhancing traits of youth such as: integrity, self discipline, loyalty and respect to successfully participate  in today's society as a Diné/Native American.

The Office of Diné Youth is dedicated to insuring that Navajo Youth under 21 years of age benefit from youth services available to them and that the services be provided in a manner that develops positive attitudes, enhance positive self-image, confidence, maturity, and pride in being a productive member of society.

Shiprock Office of Dine Youth provides prevention activities that focus on substance abuse prevention, bullying, high risk behaviors, character development, dating violence, leadership, life skills, and team building.We are an after school program/youth development center. We serve in the capacity of what is a Boys & Girls Club that is off the reservation.

The Partnership provides:

• prevention programming for youth and families
• raises the awareness of prevention efforts
• supports community planning activities and projects
• serves as a forum for community input, networking and resource sharing
• administers local and statewide programs.

Those who benefit from the Partnership are community agencies and organizations, government and private business, schools, families, and the community at large.

Prevention activities work to educate and support individuals and communities to prevent the use and misuse of drugs and the development of substance use disorders. Substance use and mental disorders can make daily activities difficult and impair a person’s ability to work, interact with family, and fulfill other major life functions. Mental and substance use disorders are among the top conditions that cause disability in the United States. What do we do?

The HCAP helps people who do not have medical insurance with their medical bills. There is an application process. The SJC HCAP provides assistance for medical, dental, prescriptions, mental health, and behavioral health.

Individuals may apply directly to San Juan Regional Medical Center or PMS/Farmington Community Health Center.

To qualify, you must meet these criteria:

• You must apply within 180 days of incurring your medical bills.
• You must be a resident of San Juan County for at least 90 days.
• Non-citizens can apply but must have "Legal Permanent Resident" status.
• There is an income limit - see HCAP Program Information for details.
• There is a limit on your personal assets - see HCAP Program Information for details.
• You must have applied for Medicaid and not qualified (we can help you with these forms)

ARISE Sexual Assault Services provides free emergency services for individuals who experience sexual assault in and around Quay, De Baca, Roosevelt, Curry, Lea, Harding, and Union counties.

We operate a 24/7 crisis line as well as provide community education to prevent sexual assault. ARISE employs several licensed therapists to benefit the mental health of clients impacted by sexual assault. We employ three SANE nurses who perform sexual assault nurse exams at various satellite locations or at our main office in Portales, NM. Education/Prevention services are available either at our physical office or at the location of the requesting organization.

At PMS, we offer a wide range of therapy and treatment options. We serve children, adolescents, adults, couples, and families. Our clients come from all walks of life and all diversities. Some of our common services include providing individual, couples, family and group therapy for mental health and substance use disorders, medication evaluation and management, and crisis intervention.

PMS provides specialty behavioral health programs based on local community need. These include specialty court services, Multi-Systemic Therapy (MST), Assertive Community Treatment (ACT), Psycho-Social Rehabilitation (PSR), Comprehensive Community Support Services (CCSS), Behavioral Management Services (BMS), Respite, Medication Assisted Treatment (MAT) and Traditional Healing.
