The Self Help Center was created to assist people in the community, who wish to represent themselves in court.
The role of staff in the Self-Help Center is to provide basic, procedural information. They can not advise you what to file or how to proceed in a case, and can not provide any form of legal advice. They can provide information on other resources in the community or throughout New Mexico. Please be courteous and patient to staff and respect the limits on what they are able to do for you.
- They CAN give you court-approved forms, some forms might not be available for all legal issues.
- They CAN answer questions about what is being requested on a form.
- They CAN tell you what certain legal words mean.
- They CAN give you information on a court case, unless it is not available by law.
- They CAN give you general information on court rules, procedures and practices.
- They CAN give you court calendars and information on how to schedule a hearing.
- They CAN give you information about how to act in the courtroom.
- They CANNOT recommend an attorney, but can give you a listing of all the practicing local attorneys.
- They CANNOT tell you what to say in court.
- They CANNOT give you an opinion about what will happen with your case or what the judge might do.
- They CANNOT give you advice about whether you should file a case or whether you should take any particular action in a case.
- They CANNOT talk to the judge for you or let you talk to the judge.

WHO/WHAT WE ARE
Vdex Diabetes is a group of medical providers focused on better control of blood sugar in people with diabetes and prediabetes. It is the failure to adequately control blood sugar that leads to the complications of the disease, and those complications are painful, expensive and ultimately, life-threatening.
A HIGHER STANDARD
As we, the Vdex Founders, surveyed the landscape of diabetes care in 2015, one point became clear: there were lots of treatments and surprisingly poor results. We set out to do better. We could see the obvious need for a higher standard of care if one were to truly “control” the disease and its effects.
A DIFFERENT MODEL
We also noticed flaws in the model through which care was delivered to patients. That model left patients uninformed, with inadequate control and unsatisfied. Doctors were overburdened dealing with a burgeoning patient population experiencing poor outcomes and increasing complications. Insurance companies were facing ever-increasing costs of care with decreased levels of patient satisfaction. It was a perfect storm of negativity. It had to change.
FRESH THINKING – PHYSICIANS AND NONPHYSICIANS
Vdex brought fresh thinking to the problem. The Founders included both physicians and as important, nonphysicians. Each started from scratch. Instead of asking, “How do we improve the current system?” we asked, “What is the best system to produce the best results for patients?” We then built that.
DIFFERENT THERAPEUTIC AGENTS
We looked at the many therapies available to control blood sugar and asked, “Which work best?” Surprisingly, many of the therapies being used were little more than bandaids on the problem. They really didn’t get the job done long term, and worse, may have created a false sense of security in patients.
EARLY RESULTS ENCOURAGING
From early 2016 we began rigorous testing of our thinking both in terms of the therapies used and the process of delivering care. The early results were better than we had dared to hope. We saw patients getting better control of their blood sugar, with less effort, with greater satisfaction, greater compliance, and greater safety. This spurred us to go further.
THE VDEX STANDARD – 6.2
The Vdex Standards for control of diabetes are higher than the industry standards. While the American Diabetes Association target for blood sugar control is an HbA1c of below 7, we at Vdex strive for a target of 6.2 and below. Why? At 6.2 and below, most patients will have minimal to no complications from the disease.
While the ADA encourages providers to “minimize” hypoglycemia, the most feared short-term complication of treatment, Vdex providers are guided to “eliminate” hypoglycemia.
THE DAWN OF A NEW ERA OF TREATMENT
At Vdex we believe we sit at the dawn of a new era in the control of diabetes. We believe, for many, if not most, patients, we can so control the disease that it can be reduced from being a killer to a mere nuisance. We believe diabetes care can go from being one of medicine’s great failures, to a resounding success.

Our Mission

Why We Developed This Program
The Adult Inpatient and Outpatient program that provides individualized treatment for adults who suffer from emotional, behavioral or mental health disorders.
The inpatient unit offers acute inpatient treatment for individuals with a primary psychiatric diagnosis. The outpatient program provides an intensive outpatient treatment Monday through Friday in the least restrictive environment and patients are able to return to their homes each evening. Medication Management Services are also offered.
Who Should Come
Anyone 18 years or older whose quality of life has been diminished by physical or emotional stressors.
The person who has not progressed or benefited sufficiently in less intensive treatment settings.

Treatment modalities are designed to meet the unique needs of adults by educating patients, families, and caregivers regarding mental health issues affecting daily living.
Treatment is supervised by a Psychiatrist and provided by a team that includes Masters level clinicians and mental health providers.
Caregivers, families and primary care physicians are encouraged to participate in the process by assisting with the identifications of problems and solutions.

Any concerned individual may refer themselves or someone else to the program. An initial clinical assessment is provided at no cost to determine the need for physician evaluation.

If you or a family member feel overwhelmed, unable to cope, extremely anxious, or have thoughts of wanting to harm yourself or someone else, please come to the Emergency Department.

A Community Health Worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served.
Community Health Workers become a part of the patient's care team by addressing any challenges they face with regard to their own health or that of their family.
All community members are served. They do not have to be a patient of GCRMC.

About this program
The Community Services Block Grant (CSBG) provides core funding to reduce poverty, revitalize low-income communities, and to empower low-income families and individuals to become fully self-sufficient. The Community Service Block Grant is awarded by the New Mexico Human Services Department to SNMCAC for Chavez, Eddy, Lea, Lincoln and Otero counties.
The CSBG program can help low-income families and individuals meet their obligations and put them on the path to stabilization.
As funding allows, the program offers emergency help in a variety of forms. Low income families can receive rent or mortgage assistance, prescription assistance, utility bill assistance and more. Clients will also have access to information and referrals. Families FIRST is a case management program of the New Mexico Department of Health, Public Health Division, that provides perinatal case management to Medicaid eligible pregnant women and children 0-3 years old. See website for information about this program. Clients receive the following services:
-A case intake to ascertain the client’s eligibility and to gather information for the purposes of care coordination;
-Presumptive Eligibility and/or Medicaid On-Site Application Assistance;
-An assessment of the client’s medical/psychosocial strengths and risk factors;
-A plan of care that links clients with resources that address the problems/needs identified;
-Standardized health education and literature related to pregnancy, birth, newborn care and child development;
-Referrals to appropriate community resources;
-Monitoring and follow-up with clients to ensure that they receives the recommended services;
-A reassessment to evaluate the status of problems identified, and to identify additional areas of concern or risk. -

Four C Inspire is an in-school mentoring program the helps students discover their value, find purpose, and build strength. Using volunteers and a well-deigned program with a proven track record, this program has helped high-risk or traumatized students succeed in school. Four C Inspire was a valuable partner to Aztec High School after the tragic 2017 in-school shooting and the death of two students.

Our club participates in a variety of projects throughout the year that involve fundraising, community service, and assisting children locally and worldwide. Among their many programs are: Dining with the Dead, Clothes for kids, Coats for kids, Shoes for kids, and the Scholastic Book Fair Program.

The Volunteer Incentive Program partners with local youth organizations, including athletic, academic, religious, and social groups, by donating $10 per student per hour spent volunteering (up to $600 per organization per year). The youth organizations choose their own volunteer activity and request approval from VIP. Upon completion, they are compensated out of our General Fund, which is funded by our donor pool. Donors also have the option to designate their contribution for specific youth organizations, in which case the maximum amount available can be raised (great for larger organizations!)

It’s My Very Own (IMVO), a faith-based, voluntary organization, seeks to make kids’ lives a little brighter by working with communities to provide displaced children with Bags of Love that meet their immediate needs.
Each bag of love includes a handmade quilt, stuffed animal, toys, books, and personal items. The bags are age appropriate and are delivered to the local agencies charged with removing children from unsafe homes.

In collaboration with FarmingtonMunicipal Schools, FCF helps the Homeless Liaison Program provide quality backpacks filled with school supplies and personal toiletry items to support children, identified as homeless. Thus far the program has impacted hundreds of children.

Apple Ridge Apartments are located in Farmington, NM, just 3 hours north of Albuquerque. Apple Ridge is an affordable senior living community featuring single level homes. Community features include; community center, on-site service coordinator for resident services, 24 hour emergency maintenance, refrigerated air in units, and on-site laundry and barbeque facilities.

The San Juan County Partnership Housing Assistance Program helps those who are experiencing homelessness and those in danger of becoming homeless with supportive services such as:
- Outreach to identify, coordinate and refer people and families experiencing homelessness to appropriate service providers while addressing their housing issues
- Financial assistance, which may include utility deposits, security deposits, rental assistance, and eviction prevention
Some of the various housing assistance programs include:
- Homeless prevention funds that provide assistance with paying arrears and possibly 3 months rental assistance, based on funding availability. Families need to have an eviction notice or a 3-day notice of nonpayment. The rental has to be under San Juan County’s FMR (Fair Market Rent) and has to pass inspection. There are income limits.
- Rapid Rehousing funds provide assistance with security/utility deposits and possibly 3 months rental assistance based on funding availability. Families have to be considered literally homeless, for example, coming from shelters, DV shelters or living in uninhabitable living places. There are income limits.
- CDBG funds through the City of Farmington provide security/utility deposits and first month rent, or pro-rated month, depending on when families move into the rental. Families have to be considered literally homeless. With this funding, HUD income limits can be exceeded up to a certain amount. This funding only serves clients within the city limits of Farmington.
- Permanent Supportive Housing (PSH) can provide security/utility deposits and 6 months rental assistance. The Head of household has to be considered disabled and chronically homeless. Chronically homeless means that the head of household has been continuously homeless for 1 year or has had 4 episodes of homelessness within 3 years that total 12 months. This funding has income limits.

Four Corners Home for Children serves people in the Four Corners region of New Mexico, Arizona, Utah, and Colorado. Since 1953, we have helped meet the physical, emotional, educational, and spiritual needs of Navajo children and their families. Our mission as an organization is to help those we serve through programs that instill hope, restoration and Christian values.

The New Mexico Home Fund helps renters and homeowners in New Mexico pay for their rent, mortgage, and utility bills. The program is funded by the Federal Government and administered by the State of New Mexico. The program brings two together the Emergency Rental Assistance Program (formerly RentHelpNM.org) and the Homeowner Assistance Program.
This NM Home Fund provides assistance for people who need some extra help due to the COVID-19 pandemic. Provided assistance does not need to be repaid. It provides support to households so that they can stay in their homes during difficult times.
Over $200M is available to support NM Renters and $55M is available to support homeowners. Both programs will run until September 2025 or until program funds are exhausted.

Farmington Preschool Academy West is a NM Public Education funded (NM PreK) site that serves Farmington students who are typically developing as well as including students with IEPs during their year immediately preceding kindergarten. Students must be four years old on or before August 31st of the current year and reside in Farmington Municipal School District. Waivers are considered on a case-by-case basis. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

The 5-Actions Program™ includes over 100 videos, 3-5 minutes in length, organized into learning modules. In total, there is about 6.5 hours of engaging content. Most videos also include links to additional resources such as TED Talks, books, and downloadable tools.
The program has extensive self-screening and assessment tools to help you identify problems with substance use and/or behavioral addictions in your life. The tools are not diagnostic, but may help you identify issues that contribute to your ongoing struggles. And if indicated, they will encourage further evaluation by treatment professionals.
It also includes access to free monthly webinars. Learn about emerging research in addiction, getting started using NM 5-Actions, indigenous healing practices, help for a loved one struggling with addiction, and more. The on-demand webinar library lets you listen and learn anytime at your convenience.

Shiprock Office of Dine Youth provides prevention activities that focus on substance abuse prevention, bullying, high risk behaviors, character development, dating violence, leadership, life skills, and team building.
Our prevention program provides community-based and family oriented alternatives for troubled and/or "high risk" youth through direct services and network systems that foster healthy and positive growth. Our goal is to identify positive alternatives to troubled and/or "high" risk youth and their families through various services and to emphasize and encourage positive family and social interaction.

The Partnership provides:
- prevention programming for youth and families
- raises the awareness of prevention efforts
- supports community planning activities and projects
- serves as a forum for community input, networking and resource sharing
- administers local and statewide programs.
Those who benefit from the Partnership are community agencies and organizations, government and private business, schools, families, and the community at large.
Prevention activities work to educate and support individuals and communities to prevent the use and misuse of drugs and the development of substance use disorders. Substance use and mental disorders can make daily activities difficult and impair a person’s ability to work, interact with family, and fulfill other major life functions. Mental and substance use disorders are among the top conditions that cause disability in the United States. What do we do?
The HCAP helps people who do not have medical insurance with their medical bills. There is an application process.
To qualify, you must meet these criteria:
- You must apply within 180 days of incurring your medical bills.
- You must be a resident of San Juan County for at least 90 days.
- Non-citizens can apply but must have "Legal Permanent Resident" status.
- There is an income limit - see HCAP Program Information for details.
- There is a limit on your personal assets - see HCAP Program Information for details.
- You must have applied for Medicaid and not qualified (we can help you with these forms)

The FIT Program provides supports and services to children who have or who are at risk for a developmental delay, and their families. Throughout the years, the delivery of early intervention has evolved to family guided services supporting your child’s learning and development in their natural environment. Staff partner with families to develop goals and strategies that are easily integrated into the family’s daily routines.

Customized Community Supports
The service focuses on maximizing independence, creating interpersonal relationships, establishing natural supports, and participating in the local community. These supports are based on individual choice of outcomes and then designed to measure progress specified in their Individual Support Plan.
Project Shield assists individuals with disabilities to increase economic independence, self-reliance, and growth within a career. Based on the individuals' career plan, they will work with a job coach during the job search process, the application and interview process, and job improvement and growth. We provide job coaches, transportation, assistive technology, and specialized job training.

- Early Head Start: This federally funded program promotes academic readiness by helping children through their social and cognitive development by employing educational, health, nutritional, social, and other services. The program also helps parents reach their full potential for their personal, educational, or vocational goals. Early Head Start is available to pregnant women and their children between the ages of 6 weeks and 3 years old.

Our shelter for battered adults is open 24/7 and provides temporary shelter for those adults and their children who are victims of domestic violence. Advocates are always available to provide service to those in need of our assistance or those who simply need a friendly voice to talk with.
Our Juvenile Non-violence program is available to those teens who have issues of violence and abuse in their lives whether or not they are involved in the Juvenile Justice System. B.E.A.R (Becoming Empowered, Accountable and Responsible) is a 16 session program for ages 13-17 which offers skills and tools to help those young people take control of their behavior and their lives.

Los Pasitos provides Early Intervention services to children ages birth to three years, that are at risk for, or that have a developmental delay or disability, get the support they need to reach their full potential. Los Pasitos partners with families to nurture and advocate for their child’s development through coaching and modeling in their natural environment so that parents and caregivers can shine while helping their child reach their full potential. Services include Occupational Therapy, Physical Therapy, Speech Therapy, Developmental Instruction, Family Counseling, and Transition to Preschool.
