The San Juan County Partnership Housing Assistance Program helps those who are experiencing homelessness and those in danger of becoming homeless with supportive services such as:
- Outreach to identify, coordinate and refer people and families experiencing homelessness to appropriate service providers while addressing their housing issues
- Financial assistance, which may include utility deposits, security deposits, rental assistance, and eviction prevention
Some of the various housing assistance programs include:
- Homeless prevention funds that provide assistance with paying arrears and possibly 3 months rental assistance, based on funding availability. Families need to have an eviction notice or a 3-day notice of nonpayment. The rental has to be under San Juan County’s FMR (Fair Market Rent) and has to pass inspection. There are income limits.
- Rapid Rehousing funds provide assistance with security/utility deposits and possibly 3 months rental assistance based on funding availability. Families have to be considered literally homeless, for example, coming from shelters, DV shelters or living in uninhabitable living places. There are income limits.
- CDBG funds through the City of Farmington provide security/utility deposits and first month rent, or pro-rated month, depending on when families move into the rental. Families have to be considered literally homeless. With this funding, HUD income limits can be exceeded up to a certain amount. This funding only serves clients within the city limits of Farmington.
- Permanent Supportive Housing (PSH) can provide security/utility deposits and 6 months rental assistance. The Head of household has to be considered disabled and chronically homeless. Chronically homeless means that the head of household has been continuously homeless for 1 year or has had 4 episodes of homelessness within 3 years that total 12 months. This funding has income limits.

Four Corners Home for Children
Four Corners Home for Children serves people in the Four Corners region of New Mexico, Arizona, Utah, and Colorado. Since 1953, we have helped meet the physical, emotional, educational, and spiritual needs of Navajo children and their families. Our mission as an organization is to help those we serve through programs that instill hope, restoration and Christian values.
Four Corners Home for Children operates a campus school, called Four Corners Academy for Excellence. The school serves students in kindergarten through eighth grade and is taught by our Director of Education and her Educational Assistant.

The New Mexico Home Fund helps renters and homeowners in New Mexico pay for their rent, mortgage, and utility bills. The program is funded by the Federal Government and administered by the State of New Mexico. The program brings two together the Emergency Rental Assistance Program (formerly RentHelpNM.org) and the Homeowner Assistance Program.
This NM Home Fund provides assistance for people who need some extra help due to the COVID-19 pandemic. Provided assistance does not need to be repaid. It provides support to households so that they can stay in their homes during difficult times.
Over $200M is available to support NM Renters and $55M is available to support homeowners. Both programs will run until September 2025 or until program funds are exhausted.

Farmington Preschool Academy West is a NM Public Education funded (NM PreK) site that serves Farmington students who are typically developing as well as including students with IEPs during their year immediately preceding kindergarten. Students must be four years old on or before August 31st of the current year and reside in Farmington Municipal School District. Waivers are considered on a case-by-case basis. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

Farmington Preschool Academy East enrolls students ages three to five who have a documented educational need for special education services. These students have Individualized Education Programs (IEPs) with goals that are written in the area(s) in which s/he qualifies. Peer students who model behavior and language also attend. Waivers are considered on a case-by-case basis. Our mission is to provide children and their families a safe, nurturing and fun learning environment that supports the whole child in developing the necessary skills to construct their own knowledge and be prepared for a successful transition to kindergarten.

The New Mexico 5-Actions Program™ is a new way to access care on your terms from the privacy of your home so you can start living your best life.
The 5-Actions Program™ includes over 100 videos, 3-5 minutes in length, organized into learning modules. In total, there is about 6.5 hours of engaging content. Most videos also include links to additional resources such as TED Talks, books, and downloadable tools.
The program has extensive self-screening and assessment tools to help you identify problems with substance use and/or behavioral addictions in your life. The tools are not diagnostic, but may help you identify issues that contribute to your ongoing struggles. And if indicated, they will encourage further evaluation by treatment professionals.
It also includes access to free monthly webinars. Learn about emerging research in addiction, getting started using NM 5-Actions, indigenous healing practices, help for a loved one struggling with addiction, and more. The on-demand webinar library lets you listen and learn anytime at your convenience.

The Office of Diné Youth is committed to enhancing traits of youth such as: integrity, self discipline, loyalty and respect to successfully participate in today's society as a Diné/Native American.
The Office of Diné Youth is dedicated to insuring that Navajo Youth under 21 years of age benefit from youth services available to them and that the services be provided in a manner that develops positive attitudes, enhance positive self-image, confidence, maturity, and pride in being a productive member of society.
Shiprock Office of Dine Youth provides prevention activities that focus on substance abuse prevention, bullying, high risk behaviors, character development, dating violence, leadership, life skills, and team building.We are an after school program/youth development center. We serve in the capacity of what is a Boys & Girls Club that is off the reservation.

The Partnership provides:
- prevention programming for youth and families
- raises the awareness of prevention efforts
- supports community planning activities and projects
- serves as a forum for community input, networking and resource sharing
- administers local and statewide programs.
Those who benefit from the Partnership are community agencies and organizations, government and private business, schools, families, and the community at large.
Prevention activities work to educate and support individuals and communities to prevent the use and misuse of drugs and the development of substance use disorders. Substance use and mental disorders can make daily activities difficult and impair a person’s ability to work, interact with family, and fulfill other major life functions. Mental and substance use disorders are among the top conditions that cause disability in the United States. What do we do?
The HCAP helps people who do not have medical insurance with their medical bills. There is an application process. The SJC HCAP provides assistance for medical, dental, prescriptions, mental health, and behavioral health.
Individuals may apply directly to San Juan Regional Medical Center or PMS/Farmington Community Health Center.
To qualify, you must meet these criteria:
- You must apply within 180 days of incurring your medical bills.
- You must be a resident of San Juan County for at least 90 days.
- Non-citizens can apply but must have "Legal Permanent Resident" status.
- There is an income limit - see HCAP Program Information for details.
- There is a limit on your personal assets - see HCAP Program Information for details.
- You must have applied for Medicaid and not qualified (we can help you with these forms)

Some of our common services include providing individual, couples, family and group therapy for mental health and substance use disorders, medication evaluation and management, and crisis intervention.

Roundtree Children’s Developmental Services is a contract provider with the New Mexico Family Infant Toddler (FIT) Program. The FIT Program provides supports and services to children who have or who are at risk for a developmental delay, and their families. The Roundtree team has 100+ years of combined experience working with children and families to enhance your child’s learning and development.
Throughout the years, the delivery of early intervention has evolved to family guided services supporting your child’s learning and development in their natural environment. Staff partner with families to develop goals and strategies that are easily integrated into the family’s daily routines.

Project Shield is committed to helping developmentally disabled adults in San Juan County live more meaningful lives. The goal is to provide individuals with the greatest opportunity to learn, grow, and explore their communities.
Customized Community Supports
The service focuses on maximizing independence, creating interpersonal relationships, establishing natural supports, and participating in the local community. These supports are based on individual choice of outcomes and then designed to measure progress specified in their Individual Support Plan.
Project Shield assists individuals with disabilities to increase economic independence, self-reliance, and growth within a career. Based on the individuals' career plan, they will work with a job coach during the job search process, the application and interview process, and job improvement and growth. We provide job coaches, transportation, assistive technology, and specialized job training.

- Head Start: To enroll in Presbyterian Medical Services’ Head Start center-based programming, children must turn 3 prior before August 31/September 1 of that academic year, and the maximum age is 5 years.
- Pre-K: Our Pre-K program consists of direct services to 4-year-old children who will attend kindergarten the following year, providing developmentally appropriate activities for parental involvement. New Mexico Pre-K is a voluntary program funded by the state of New Mexico. It is designed to increase access to high-quality pre-K programs, help children through developmentally appropriate services, and it focuses on academic readiness.

Our shelter for battered adults is open 24/7 and provides temporary shelter for those adults and their children who are victims of domestic violence. Advocates are always available to provide service to those in need of our assistance or those who simply need a friendly voice to talk with.
Our Juvenile Non-violence program is available to those teens who have issues of violence and abuse in their lives whether or not they are involved in the Juvenile Justice System. B.E.A.R (Becoming Empowered, Accountable and Responsible) is a 16 session program for ages 13-17 which offers skills and tools to help those young people take control of their behavior and their lives.

Los Pasitos provides Early Intervention services to children ages birth to three years, that are at risk for, or that have a developmental delay or disability, get the support they need to reach their full potential. Los Pasitos partners with families to nurture and advocate for their child’s development through coaching and modeling in their natural environment so that parents and caregivers can shine while helping their child reach their full potential. Services include Occupational Therapy, Physical Therapy, Speech Therapy, Developmental Instruction, Family Counseling, and Transition to Preschool.

- Parent Education/Home Visiting: This Early Childhood Service is designed for parents and guardians of children from birth through 5 years of age. This includes adoptive parents and teen parents, as well as expectant parents. The goals are to provide educational services and child development guidance, to ensure babies are born healthy, and to keep children safe and sound. Children’s psychological, emotional, and mental health are all addressed and parents are taught how to nurture their children through community support to ensure children are ready for school. This program is built on the belief that a parent is their child’s first and most important teacher.

We provide Sexual Assault Nurse Exams (SANE – medical/forensic exams), rape crisis advocacy, and counseling. Our staff also works in the community to prevent sexual assault through multi-session, education-based programs with specific groups. This education includes awareness programs, including ‘Safe Dates’, a healthy relationship curriculum, geared for high school students.
SAS will be will you throughout every step of your recovery process, from an exam to counseling, and facilitation of legal services to pursue the person who assaulted you. Established in 1978, Family Crisis Center (FCC) is a 501(c)(3) agency and comprehensive domestic violence service provider, offering a broad range of services geared toward response to the crisis of domestic violence. Services are provided at two facilities including Marge's Place, a protective shelter for victims of domestic violence and their children, and our Counseling Center which provides services and support to victims, child witnesses, and offenders.
Incorporating a trauma lens approach allows FCC to address the physical and emotional needs of domestic violence survivors, support the healing and recovery process, and empower survivors to take positive steps toward their own well-being and the well-being of their children.

DNA People’s Legal Services is a 501(c)(3) nonprofit legal aid organization that provides free civil legal services to low-income people who otherwise could not afford to hire an attorney. DNA provides legal assistance, advice and representation in U.S. and tribal courts, promotes tribal sovereignty, and offer community education programs that promote greater understanding of the law. The Navajo Nation Growing In Beauty program (GIB) meets the needs of Navajo children with delays or disabilities from birth to five years of age while honoring the unique culture and language of the Navajo people. Receiving and processing referrals from pediatricians, health care professionals, and other service providers.Types of service that may be provided are based on needs and preference outlined by the individualized plans. These plans may include the following: service coordination, linkage to other services for family needs, vision and hearing services, specialized instruction or special education, nutrition services, diagnostic evaluations, family training, counseling, and advocacy, other family support and respite services, occupational and physical therapy, speech and language therapy, diagnostic evaluations and developmental assessments, assistive technology, education on independent living, and transportation services.

We offer up to a year (on a case by case basis) of on campus housing to those who desire to become financially independent through full time employment, learn healthy coping skills while addressing the trauma of violence, work on obtaining the necessary skills to break the cycle of violence. The primary purpose of the Child & Family Development Center (CFDC) on the campus of San Juan College (SJC) is to serve as a laboratory school for early childhood, nursing, psychology, dental and sociology students at the College. Practicum students in early childhood education courses are part of the Center staff each semester. They are supervised by qualified teachers to plan, implement and evaluate developmentally appropriate activities for young children.
The secondary purpose of the CFDC is to provide child care and early education services. The Center is accredited by the National Association for the Education of Young Children (NAEYC). NAEYC accreditation recognizes programs that comply with nationally recognized criteria for high-quality early childhood programs. The CFDC is licensed by the State of New Mexico Early Childhood Education and Care Department.
