Birth & Death Certificates

Our Vital Records office collects, processes, corrects and issues copies of birth and death certificates.

• Our Vital Records Call Center is also available from 8:00 a.m. to 4:00 p.m. to answer your questions at 1-866-534-0051 or 505-827-0121.

• We only issue certificates for individuals who were born in the state of New Mexico. Please see the Vital Records Reference by State to find out where to obtain birth certificates for other states.

• New Mexico birth and death certificates are restricted access records. State law restricts access to the registrant, registrant's immediate family members or those who represent tangible proof of legal interest in the requested record.

• Immediate Family means any of the following: mother, father, sibling, child, grandchild, current spouse, or maternal or paternal grandparent. Paternal grandparent is eligible if father is listed on the vital record.

Gender Change Legislation

• Effective October 29, 2019, New Mexico law allows individuals who were born in New Mexico and want to change the gender designation or the gender designation of their child on the birth certificate to do so by completing the appropriate request form through the Bureau of Vital Records.
• The new law will allow for Male, Female and X as acceptable options. "X" refers to a gender other than male or female or an undesignated gender.

Medical Cannabis Program

Application Process-

• To begin the application process (new and renewal), visit your Medical Provider; they must be the one to start your application.
• As a reminder, the Department of Health does not charge any fees to enroll and participate in the Medical Cannabis or to register and use the Online Patient Portal.

Special Supplemental Nutrition Program for Women, Infants, & Children (WIC)

• The WIC Program – serves to safeguard the health of low-income pregnant, postpartum, and nursing (breastfeeding) women, infants, and children up to age 5 who are at nutritional risk by providing nutritious foods to supplement diets, information on healthy eating including nursing (breastfeeding) promotion and support, and referrals to health care.

• The New Mexico WIC Program provides free healthy foods, ideas for healthy eating and maintaining good health habits, support for nursing families, and connects families with other community services. This combination of services is designed to create a support system that encourages families to feel confident in their own ability to maintain happy and healthy futures.

Immunization Program

Food Pantry and Clothing:

Our members maintain a pantry of grocery items in order to help others who are in need.  We keep staple items such as flour, sugar, cooking oil, etc. as well as canned vegetables, fruit and meats.  You can help in this ministry by bringing in food items to stock the shelves, or in filling the bags to have them ready and waiting for those who come to our building during the week asking for aid.  The pantry also exists for our own members who have difficulties from time to time and need assistance.  Please feel welcome to share your needs with us so we may give you the help you need.  The clothing room has an assortment of gently-used clothing in many sizes and for all seasons. Help is needed to sort, label, stack and hang up donated items to make them readily available for those in need. If you enjoy organization and layout, you may find a place to be useful in this ministry.

Food Pantry is open to the community on the 2nd and 4th Saturday of every month from 9am-10am.

Electric Services and Scholarships Program

RCEC's scholarship program is generated by contributions or donations and interest from unclaimed capital credits.  Dependents of members are eligible to apply for these scholarships.  A scholarship committee meets each year in March to make selections and recipients are introduced during the Annual Meeting in April each year.  Scholarship applications are e-mailed to area high school counselors for distribution to senior students that are dependents of members of the cooperative.

Returning College Students can re-apply for a scholarship (amount to be determined according to available funds) on a yearly basis.  This application follows the graduating high school senior application.
Members can donate to the Roosevelt County Electric Education Foundation at any time.

The Pregnancy Resource Cener is a Christ-centered ministry that acknowledges that every human life begins at conception and is worthy of protection. We offer compassion, hope, and help to anyone considering abortion by presenting them with realistic alternatives.

Earn while you learn

• Earn While You Learn (EWYL) provides free prenatal and parenting classes, life skills classes and Bible studies to men and women, all focused on creating healthy families. These classes are conducted by a Client Advocate who is able to discuss the curriculum as well as answer questions. The one-on-one aspect of your program allows us to individualize classes based on the client's specific needs and situation.

  Earning Points: You can take one class per week. For each class you take, you can earn up to 15 points: 5 for arriving/answering on time, 5 for completing the homework, and 5 for participating in the follow-up conversation. The PRC will track your points for you.

  For Spanish Speakers: We offer classes in Spanish twice each month in a group setting.  All materials are printed in Spanish and the class is moderated by a Spanish-speaking client advocate.

Seniors meet to play cards, play pool, play dominoes, or just visit with each other.  The men meet on Monday afternoons for dominoes and couples meet Monday evenings for pool!  All seniors meet on Thursday afternoons for whatever game you might want to play. and everyone brings food for a potluck lunch every 2nd Thursday of each month!  All Senior Citizens in Roosevelt County, past or present, are invited to attend.

This program gives the offender a second chance while holding them accountable for their actions. There are two child development centers (CDC) located at Cannon AFB

• Cannon CDC (335 West D. L. Ingram Ave, Cannon AFB)
• Chavez CDC (101 James Boatwright Avenue, Cannon AFB)

Cannon CDC Capacity- 158 children.

Chavez CDC Capacity- 168 children.  

Both centers provide care for children 6 weeks until the start of kindergarten.
Both centers are accredited by the National Association for the Education of Young Children.  

The staff supports the development of the whole child and provides active, hand-on learning experiences to meet the individual needs of each child in the program.  To encourage children’s growth and ensure activities are developmentally appropriate, staff observes all children on a regular basis and uses this assessment in planning related activities.  Parents and staff work together to set goal for each child based on the strengths, progress and needs.

Each CDC uses “The Creative Curriculum.”  The curriculum provides progressive developmental activities in the following areas:  social, emotional, physical, cognitive and communication.  The centers have also worked with local early intervention specialists who can provide one on one support for children who may have a learning delays or conditions affecting growth and learning. 

Both CDC's are enrolled in the USDA food program and offer nutritious and appealing meals for breakfast, lunch and snacks.   The cost for child care includes all meals, including baby food and some infant formulas.  

The families and members of NAMI San Juan County NM are here to help! We offer understanding to anyone concerned about mental illnesses and the treatment of mental illness.

Mental illnesses are brain disorders that are biologically based medical problems. Untreated, they can cause severe disturbances in thinking, feeling and relating. This results in substantially diminished capacity for dealing with the ordinary demands of life. Mental illness can affect persons of any age and occur in any family. They are not caused by bad parenting and not evidence of weakness of character.

CAYA is a free 7-month Christ-centered residential treatment program located in Farmington, New Mexico, for women overcoming addictions.

Our residential treatment program provides 24-hour long-term care for 7 months with a 5-month transition to women who are 18 years of age or older, have come to a place of true brokenness, and are ready to surrender their lives to become new in Jesus Christ.

The focus is on helping the women spiritually learn or re-establish healthy functioning skills and values.

Our program also encourages the involvement of family in the journey. Since addiction and associated recovery are very much interconnected with family, we encourage them to visit during specified days and times while their loved one is in treatment.

Home Preservation

Home Preservation is a program of Habitat for Humanity of Roosevelt &Curry Counties that helps low-income homeowners by offering minor home improvements. Volunteer teams work to improve the condition of homes by painting and performing minor repairs at minimal costs to the homeowners who would be unable to complete the repairs on their own. Able-bodied homeowners are encouraged to work alongside the volunteers in a cooperative effort. This program is for homeowners earning 50% or less of the median income for the county in which they live.

Youth Homes

• Provides a loving family-like environment with a room to call your own, home-cooked meals and 24/7 care.
• Supports strong educational and extra-curricular opportunities.
• Teaches Christian values through weekly devotionals and church community.
• Gives a sense of stability and belonging.

Single Parent Homes

• Provides housing with a supportive community.
• Creates goal-oriented plans to help achieve financial stability.
• Gives children the opportunity to participate in Leadership Training in Christ, 4-H and other activities.
• Provides mentorship, counseling, and connection to additional social services.

New Mother Homes

• Escape homelessness or family crisis and sleep safely at night.
• Live in our fully furnished housing with all bills paid. This includes internet, toiletry and household supplies.
• Receive counseling and social services coordination while working on a goal-oriented plan for the future.
• Develop a connection to Christ and to a church home.

Young Adult Homes

• Serves young adults ages 18-22 by providing shelter and spiritual guidance in a Christian environment.
• Designs structure to create self-supportive, community-minded, faith-based futures.
• Provides mentorship, counseling, and connection to additional social services.
• Creates life changing opportunities through professional guidance and donor-provided scholarships.

Residential Care

• Children in our Residential Care program live in a home-like cottage setting that accommodates up to eight children. Each cottage has married Christian couples who serve as house parents, caring for the children on a 24-hour basis. Life at NMBCH is not unlike life in a typical family.
• House parents are trained and committed to helping each child grow physically, mentally, spiritually, and socially. This guidance and growth enables the child to meet personal goals and ultimately make significant lifestyle changes.
• Children attend Portales Public Schools and participate in athletics as well as other extracurricular activities through the schools. They are also involved in our 4H Club.

Adoption Services

Hope Children’s Home has rescued nearly 5,000 children between the ages of infant to 18 years of age. The type of children we care for are not juvenile delinquents, but rather children that, through no fault of their own, find themselves in a situation of needed rescue. Some children simply come out of a situation where a parent just needs a little help.

Apria Healthcare is a provider of home respiratory services and certain medical equipment, including wheelchairs, hospital beds, walkers, lifts, durable medical equipment, oxygen therapy, inhalation therapies, sleep apnea treatment, and negative pressure wound therapy.

• A residential care for older adults who need help with normal daily activities.
• Seniors in assisted living receive personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting as needed so that they’re able to live as independently as possible.

• provides expert, caring help -- No matter the situation, the specialists at 211 listen, identify underlying problems, and connect people in need with resources and services in their community that improve their lives.

Imagination Library

• Children 0-5 years-old can receive a free book delivered to their mailbox once a month through the Dolly Parton Imagination Library.

Successful You

• Closet that consists of gently-used business wear including jackets, suits, dresses, blouses, pants, and shoes. Women sent by referral will be able to select clothing items ideal for an upcoming job interview or a new job. Women are able to try on clothing by appointment only.

4-H Youth Development Program

Our 4-H Youth Development program is a dynamic non-formal education program. It provides opportunities for youth to acquire life skills and gain knowledge while having fun. The life skills learned in 4-H enable youth to become productive, well-informed, self-reliant, responsible adults.
